IOSPHERE: Application Guidelines and Instructions

Please read these instructions carefully. Please print all information clearly, cleanly, and completely. This will help us greatly.

Application Form
Fill out the application form by providing us with:

• your full name, or organization's name, mailing address, and phone number(s).
  (If there are any changes in this information, you are responsible for notifying us ASAP.)



• the service type.

(All services are renewed on a monthly basis. All prices do not include GST.)


• the method of payment

[Cash / Cheque / Credit Card (Visa/MC/Amex) / Tele-Banking / Interac]
(All new accounts are activated upon receipt of your first month's service cost.)


• any system information that may help us to evaluate your software/hardware requirements. Please consult the application form for the minimum system requirements.



• your signature and date of signature.

Service Agreement
Read the Service Agreement carefully, and initial the bottom of the document to acknowledge that you have read the agreement, and agree to abide by its terms.

Returning the forms
Once you have completed the forms, you must return the Application Form and the Service Agreement to Iosphere by fax, mail, or in person. If we are missing any pages or critical information (as indicated in bold on this page), we cannot create the account. We will contact you by phone to confirm the creation of your account, and assist you in configuring your computer, if required.
